Family Support

1. Registered Nurse Case Manager

“I am responsible for a total body assessment of how your loved one is feeling.”

  • I come at least twice a week.
  • I manage medication.
  • I take all of your loved one’s vital signs

Most importantly, I report to the Physician Medical Director (PMD) and discuss options with your family to make improvements for your loved one’s quality of life.

2. Certified Nursing Assistant

“I care for your loved one’s cleanliness and personal hygiene. I want my patients to be comfortable and feel good about themselves.”

  • I come at least twice a week.
  • I give full showers and baths.
  • I can wash and dry hair.
  • For the ladies, I can file and paint nails.
  • For the gentlemen, I can give a nice shave.
  • I can change sheets and some light tidying up.
  • I am a friend.

3. Physician Medical Directors

“I support our Registered Nurses and our entire team to ensure that your loved one is getting the best quality of life care.”

  • I will be happy to coordinate with your current Primary Care Physician.
  • I manage and prescribe medications.
  • I specialize in pain control and symptom management. I utilize the most effective medications and treatment for your loved one.
  • I host weekly interdisciplinary team meetings where we discuss every patient.
  • I and one of my associates are on call 24/7 with our nursing team.

Would you like to use your Primary Care Physician during hospice? We will gladly partner with your doctor to provide care for your loved one.

4. Chaplain

“I believe that health of the body cannot be separated from health of the soul. I am here for you and your family.”

  • I help bring your heart peace amidst the struggle.
  • I pray with you, offer emotional support, and offer bereavement support.
  • I am here to support you wherever you are spiritually.
  • I can coordinate visits with your church pastor.

Did you know that Sage Hospice offers bereavement support for up to 13 months after the loss of a loved one? We are there for you for the first of everything: the first Christmas, the first birthday, the first anniversary of your loss.

5. Medical Social Worker

“I remove the logistical stress, arrange support, and help find benefits for your loved one.”

On your own, it is very overwhelming for a family to review paperwork and make calls amidst a health crisis. We take care of this for you. I sit with you at your kitchen table with you to plan for hospice care for your loved one. For which benefits do you qualify? VA Aid and Attendance? Arizona Long Term Care (ALTCS)?

  • Benefits – For what benefits does your loved one qualify? State Aid and Veterans Benefits?
  • Advanced Directives – Does your loved one have an end of life plan? What do or do you not want in an emergency?
  • Counseling – Entering hospice care is not easy. We help talk through difficult realities.
  • Funeral Planning – How should we make funeral arrangements?
  • Power of Attorney – We help you navigate and process paperwork.
  • Referrals – We coordinate with other agencies for additional support. (e.g. mobile dentistry, Meals on Wheels)

6. Volunteers

“We bring an extra level of companionship and compassion into your home.”

My favorite moments with my patients are:

  • Sharing stories from childhood
  • Playing cards and boardgames
  • Putting together meals
